The links above will take you to my General Income Tax Return Worksheet. They are in Microsoft Word and Adobe PDF formats. Click a link to download the format of your preference. For your convenience, the Microsoft Word Form (PC or Mac) of my worksheet can be filled out on your own computer.

In the Microsoft Word Form, click the field you want to fill out (it appears as a gray box on the form) and type in the information. The Tab key will advance your cursor to the next field or you can manually click any field. If the field is a check box, click the appropriate box and an "X" will appear in the box. If you made a mistake with the check box simply click the box again to remove the "X".

Once the worksheet has been completed, save the Word document as your name (i.e., DoeJane.doc), and attach the document to an e-mail you send to me. You may also scan your 2008 tax reporting documents and email them to me as PDF files.

If you prefer, you may also mail or fax the worksheet to me along with photocopies of your W-2s, 1099s, K-1s., brokerage statements, etc. Do not send original documents to me in the mail without first making copies of them.

If you have already prepared other reports with the appropriate information I need to prepare your tax returns, it is not necessary to duplicate the information on the worksheet.

An organizer is also available that is created by my tax preparation software. It includes your 2007 tax information, with space to write in your 2008 numbers. If you would like this organizer, please contact my office and request it. We will be happy to mail or email it to you.

Once you have completed the worksheet and assembled your tax information, you have several options:

Make an appointment to come to my office. Bring your completed Worksheet along with your W-2s, 1099s and other tax statements. If you are a new client, bring a copy of your most recent tax return. I'll mail you your completed returns or you can come pick them up in person.

Electronic Filing

If we are filing your returns electronically, I will email you a copy of your return as a PDF document. Accompanying your return are signature authorization forms for you to sign and return to me. IMPORTANT. Before I can transmit your returns electronically, I am required by law to have these signed signature forms in my office. This is a step that is often overlooked. You can mail/email/fax these signatures forms to me, along with payment of my invoice. The sooner I have your signed forms, the sooner you can receive your refund.

If we are filing your returns by paper/mail, I will mail the completed returns to you for your signature along with filing instructions. You may also come and pick up your completed returns in person at my office.

It is important that you get your information to me as soon as possible if you want to file by April 15. If I receive your materials after March 31, I may need to file an automatic extension for you. This extension would expire on October 15, 2009. Please note that this is an extension of time to file, not an extension to pay any tax due. If you owe tax with your return, interest and late penalties may apply even if I file an extension for you.
