Online Submission of the Worksheets

I provide two worksheets that help to organize your income information. These worksheets are very helpful to me as I prepare your income tax return. One worksheet is designed specifically for performing artists. The other worksheet is more generic for those persons not in the performance art industry. Those of you with tax issues not covered by the worksheet may need to provide me with additional information. Feel free to call me with questions while completing the worksheet. Include additional pages and make notes where necessary. If you are married, it will usually be better to complete the income and expense sections of the worksheet separately.

Once you have completed the worksheet and assembled your tax information, you have several options:

•	E-mail the completed Word form Worksheet to me at david@davidturrentine.com. You may also attach scans of your W-2s, 1099s and other tax statements to the e-mail or send photocopies of them to my office by snail-mail. Do not mail original W-2s without making copies. They are difficult to replace if lost in the mail. I will mail your completed returns to you for your signature along with filing instructions.

•	Snail-mail the completed Worksheet to me with photocopies of your W-2s, 1099s and other tax statements. Do not mail original W-2s without making copies. They are difficult to replace if lost in the mail. I will mail your completed returns to you for your signature along with filing instructions.

•	Fax a completed Worksheet to me along with your W-2s, 1099s and other tax statements. I will mail your completed returns to you for your signature along with filing instructions.

•	Make an appointment to come to my office. Bring your completed Worksheet along with your W-2s, 1099s and other tax statements. If you are a new client, bring a copy of your most recent tax return. I'll mail you your completed returns or you can come pick them up in person.
